* Members must have previously paid their membership fee and must be registered before 1 July 1999, otherwise a reduced fee will be charged.** Registration forms must be accompanied by a letter of confirmation from the Head of Department.
The latest date for pre-registration will be 17 August 1999. Registrations received after this date will be processed on-site.
The registration fee for delegates includes:
- Admission to the scientific sessions and the exhibition
- Meeting documentation including the Book of Abstracts
- Opening Ceremony and Civic Reception on Sunday 5 September 1999
- Civic Reception and Concert on Monday 6 September 1999
- Coffee/tea during breaks
The registration fee for accompanying persons includes:
- Opening Ceremony and Reception on Sunday 5 September 1999
- Civic Reception and Concert on Monday 6 September 1999
- One half-day tour to Loch Lomond (If the box on the registration form is not ticked, the tour will not be considered booked)
- Admission to the Presidential Address, Honoured Guest and Basic Science Lectures
ONLY FULLY REGISTERED DELEGATES WILL BE ADMITTED TO THE SCIENTIFIC SESSIONS. DELEGATE FEES DO NOT INCLUDE OPTIONAL TOURS OR HOTEL FEES.
PAYMENT
- by credit card--American Express, Visa, Mastercard will be charged in Sterling. Please note that a surcharge of 2% (Visa and Mastercard) or 2.5% (American Express) will be levied on credit card bookings.
- by cheque--in EUROs drawn on a UK bank, made payable to: The European Association for Cardio-Thoracic Surgery
- currency--currencies accepted for payments on-site will be Pounds Sterling and US Dollars.
CANCELLATION
Cancellation must be made in writing and received before 1 August 1999, when the fees, less a 20% administration charge, will be refunded. It is regretted that after this date no reimbursement of fees can be made.
