REGISTRATION AND PAYMENT
  Pre-registration is closed.  You are welcome to register on-site.

REGISTRATION
Registration is available by completing and returning the   Registration Form or by registering online.
Registration fees are as follows:
Cardiac Programme (08.00-17.30)
Early fee, until 2 July                                        €250
Late fee, 3 July - 27 August                    €350
On site fee, applied from 28 August                €400
Reduced fee for residents, nurses           €50 (€75 from 28 August)

Thoracic Programme (13.00-17.30)
Early fee, until 2 July                                       €100
Late fee, 3 July - 27 August                              €150
On site fee, applied from 28 August                €200
Reduced fee for residents, nurses           €50 (€75 from 28 August)
Registration will be available at the Congress Center Leipzig from 14.00-18.00 on Friday 10 September and from 07.00-14.00 on Saturday 11 September.
PAYMENT
Registration payments should be made either online or on the pre-registration form and sent to the EACTS Executive Secretariat. All payments must be in Euro (€) and should be made as follows:
  1. Credit Card - VISA, Mastercard and American Express are accepted.
  2. Bank Transfer - Transfers made must be exempt of charges to EACTS Trading Company Ltd. In order to ensure that your payment is identified, you must mention the name of the delegate and "3rd EACTS / ESTS Joint Meeting".

Transfers must be exempt of charges to EACTS Trading Company Ltd. In order to ensure that your payment is correctly identified, you are reminded to include the name of the delegate and 'Techno-College' in the bank transfer. PERSONAL CHEQUES ARE NOT ACCEPTED. The only currency accepted for payments on site will be EURO (€).


ACKNOWLEDGEMENT AND CONFIRMATION OF REGISTRATION

Acknowledgement and confirmation of registration will be sent by email. Registration will not be confirmed, however, until payment has been received. Where no email is given, acknowledgements and confirmations will be by ordinary mail.


CANCELLATION

Cancellations must be made in writing to the EACTS Executive Secretariat and received before 13 August 2004. The fee(s), less a 25 % administration charge, will be refunded. It is regretted that after this date no reimbursement of fees can be made.
